At Eternal Hill Hospice, consider us grateful to be able to help you through this difficult time. We try not to focus on the loss through death, but to celebrate the closing of one's life with dignity.
Our team of experts strive to meet the specific needs of our patients and their families. Eternal Hill Hospice work in coordination with our patients to deliver quality care

Doctor
Our team is lead by the Hospice Medical Director, who may or may not make house calls. However, through our collaboration with the case manager, the medical director will ensure that the correct medical care is administered.

Nurse
A registered nurse will be assigned to each patient as the case manager. The Case Manager will ensure that all needs of the patient and family are met through education and support. The nurse will make weekly visits as deemed necessary to patient and family needs.

Certified Nursing Assistant
The Certified Nursing Assistant helps our patients with their activities of daily living which includes bathing, dressing, feeding, shaving, and other personal care on a case by case basis.

Social Worker
The medical social worker provides necessary information to the patient and family dealing with a terminal illness. Information may include, but is not limited to, finances, housing, VA services, other insurance services, and assistance with meals on wheels. The medical social worker is your community liaison, a wealth of knowledge in community resources and most of all, a great listener.

Chaplin/Bereavement Counselor
A spiritual counselor based on your individual needs. They will provide spiritual guidance and support to you and your family based on your needs. Also, they will provide resources as needed for each individual patient's and family's spiritual beliefs, as necessary. They are a part of your support system, as well as great listeners. They will also provide education and support during the grieving process and post death. Bereavement support will last up to 13 months after death.

Volunteers
Volunteers usually provide extra care, as needed, such as companionship and caregiver breaks. Support includes reading, grocery runs, etc., but no hands-on care.
